We are currently seeking a dedicated and compassionate Occupational Therapist (OT) to provide services in both Skilled Nursing Facility (SNF) and Home Health settings throughout Iowa. This position offers the opportunity to help patients regain independence, improve daily living skills, and enhance their quality of life through individualized, patient-centered care. Position Overview The Occupational Therapist will evaluate and treat patients experiencing physical, cognitive, developmental, or psychosocial limitations that impact their ability to perform activities of daily living. Working collaboratively with interdisciplinary healthcare teams, the OT will develop and implement treatment plans designed to maximize patient independence and functional outcomes across SNF and home health environments. Responsibilities Conduct comprehensive occupational therapy evaluations and assessments. Develop individualized treatment plans based on patient needs, goals, and physician orders. Provide therapeutic interventions to improve functional mobility, self-care, cognitive functioning, coordination, and daily living skills. Assess and recommend adaptive equipment, assistive technology, and environmental modifications. Educate patients, families, and caregivers on treatment strategies and home exercise programs. Collaborate with physicians, nurses, physical therapists, speech-language pathologists, and other healthcare professionals. Maintain accurate and timely documentation in compliance with state, federal, Medicare, and facility regulations. Monitor patient progress and modify treatment plans as appropriate. Participate in care planning meetings and interdisciplinary team discussions. Qualifications Graduate of an accredited Occupational Therapy program with a Bachelor's, Master's, or Doctoral degree in Occupational Therapy. Current Iowa Occupational Therapist License issued by the Iowa Board of Physical and Occupational Therapy. Current CPR/BLS certification or ability to obtain prior to start. Valid driver's license, reliable transportation, and current auto insurance for home health travel. Strong clinical, communication, and documentation skills. Experience in Skilled Nursing, Rehabilitation, Long-Term Care, or Home Health settings preferred but not required. New graduates are encouraged to apply.
