In this role, you will assess, plan, and implement individualized therapy programs to help patients regain function, independence, and quality of life. You will collaborate closely with physicians, nurses, and other therapists to deliver patient-focused, evidence-based care. Responsibilities Perform comprehensive occupational therapy evaluations focusing on functional abilities, ADLs, cognition, and safety. Develop and implement individualized, evidence-based treatment plans to improve function, independence, and quality of life. Provide hands-on therapeutic interventions including ADL training, therapeutic exercise, neuromuscular re-education, and functional mobility training. Recommend, train, and educate patients in the use of adaptive equipment and assistive technology as appropriate. Educate patients, families, and caregivers on home programs, safety strategies, energy conservation, and compensatory techniques. Document evaluations, goals, progress notes, and discharge summaries accurately and timely in the electronic medical record. Collaborate with physicians, nurses, rehabilitation staff, and case managers to coordinate comprehensive, patient-centered care plans. Participate in interdisciplinary care conferences and discharge planning to support safe and effective transitions of care. Adhere to all regulatory, licensing, and professional practice standards, including infection control and hospital policies. Contribute to quality improvement initiatives, staff education, and professional development activities within the department.
