ABOUT THE ROLE

Are you a dedicated Occupational Therapist passionate about empowering individuals with disabilities to live independently in their communities? We are seeking a qualified Occupational Therapist (OT) to join our Personal Care Management (PCM) team. In this vital role, you will perform home-based clinical evaluations and reevaluations for MassHealth members applying for or continuing in the Personal Care Attendant (PCA) Program. Working in collaboration with our nursing team, you will evaluate members' functional abilities and determine medical necessity for physical assistance with Activities of Daily Living (ADLs) and Instrumental Activities of Daily Living (IADLs).

KEY RESPONSIBILITIES

Perform Comprehensive Evaluations:

Conduct in-person evaluations and annual reevaluations in members' homes across the local community.

Evaluate Functional Needs:

Assess physical assistance needs for key ADLs (mobility, bathing, dressing, eating, toileting, passive ROM, medication assistance) and IADLs (meal prep, housekeeping, adaptive equipment maintenance).

Collaborate Multidisciplinary Care:

Work closely with Registered Nurses (RNs) and Licensed Practical Nurses (LPNs) as a core member of the MassHealth evaluation team.

Support Member Self-Direction:

Assist in evaluating members' capacity to direct their own care or determine the need for a surrogate/administrative proxy.

Maintain Timely Documentation:

Complete and submit required MassHealth evaluation forms and clinical documentation in a professional and timely manner.

QUALIFICATIONS

Licensure:

Currently licensed by the Massachusetts Board of Allied Health Professionals.

Certification:

Currently certified by the National Board for Certification in Occupational Therapy (NBCOT).

Experience:

Experience in home health, community care, adult/pediatric rehabilitation, or disability services preferred.

Regulatory Knowledge:

Knowledge of MassHealth PCA regulations (130 CMR 422.000) is a plus.

Transportation:

Reliable transportation for home visits across the designated service area.
