Job Description

Alta Healthcare Services is seeking a Maryland-licensed Occupational Therapist to conduct in-home environmental assessments for Medicaid clients who require home modifications.
Responsibilities:
Evaluate clients' functional and accessibility needs. Assess home safety, mobility barriers, and fall risks. Recommend medically necessary modifications and adaptive equipment. Complete clear, timely Medicaid-compliant assessment reports. Coordinate with clients, caregivers, contractors, and care teams as needed.
Qualifications:
Active Maryland Occupational Therapist license. Experience with home safety, accessibility, aging in place, or disability assessments. Strong documentation and report-writing skills. Reliable transportation for home visits.
