Job Description

PowerBack Rehabilitation seeks a patient-centered Occupational Therapist to join our Healthcare & Medical Services team. In this role, you will evaluate post-hospital patients, develop individualized treatment plans, and deliver intensive, evidence-based interventions to restore function and independence after surgery, injury, or illness. You will collaborate closely with an interdisciplinary team in a supportive, growth-focused culture, using advanced rehab practices and technology. Responsibilities include ADL retraining, caregiver education, discharge planning, and thorough EMR documentation while championing compassionate, goal-driven recovery. Responsibilities Evaluate post-hospital patients to identify functional deficits and rehab needs Develop and implement individualized, evidence-based treatment plans Provide therapeutic interventions to improve mobility, ADLs, and safety Educate patients and families on home programs and adaptive techniques Document evaluations, progress notes, and discharge summaries in EMRCollaborate with physicians, nurses, and rehab team to optimize outcomes Participate in care conferences and contribute to discharge planning Support a culture of continuous improvement and clinical excellence Required Skills Patient assessment and evaluation Treatment planning and goal setting
