- We are seeking Licensed Occupational Therapist to fill a Home Health positions in lake Orion and surrounding areas .
Job Responsibilities:
Assist patient in learning appropriate exercises and treatments to reach the patient goals for improvements in movement and function. Perform thorough evaluations of patients' functional status, including muscle function, endurance, visual coordination, written and verbal communication skills and ADL and IADL performance. Assess cognitive, psychological and motor aspects of performance and develop individualized intervention plans to achieve favorable outcomes Records clinical and progress notes at the time of the visit to ensure continuity of qualitative client care in a timely basis. Re-evaluates and records the clients care needs at each home visit Continually assess and revise the occupational therapy care plan, and participate with care team in the multidisciplinary care plan, as appropriate. Educate and instruct patients, family members, or other patient representatives, in rehabilitative care and activities necessary to promote the client's health, safety and independent living.Job Requirements:
A current license as an Occupational Therapist in Michigan A graduate of an occupational therapy program from accredited college- New Grads Welcome to Apply
