Legacy Home Care and Home Health Care, Inc. are hiring an OTR/L to provide home-based therapy throughout Southern and Southwest Minnesota . This is an opportunity to step into a role with clinical independence, one-on-one patient care, territory-based autonomy, and the ability to build treatment around what you believe will actually help your patient. Assess. Decide. Treat. Adapt. Home health requires strong clinical judgment. You won't have a therapy gym full of equipment or another therapist standing ten feet away. You'll walk into a patient's home, evaluate what's happening, identify the barriers keeping them from functioning independently, and determine the best way forward. That might involve ADL retraining, adaptive equipment, caregiver education, environmental modifications, strengthening, cognitive strategies, fall prevention, or an entirely different approach. A nurse may identify that a patient is struggling at home. A PT may recognize a mobility concern. A physician may order therapy. You're the one evaluating how those challenges affect the patient's ability to actually live their life. Can they dress themselves? Can they safely complete a shower? Can they prepare a meal? Is cognition affecting their ability to manage everyday tasks? Does the home need to be modified? Is the caregiver providing the right amount of assistance? Your recommendations can directly influence whether a patient is able to function safely and independently at home. What You'll Do Complete in-home OT evaluations and reassessments Develop individualized plans of care and functional goals Address bathing, dressing, grooming, toileting, meal preparation, mobility, and other ADLs Evaluate strength, coordination, cognition, fine motor skills, safety, and functional performance Complete home safety and fall-risk assessments Recommend adaptive equipment and environmental modifications Help patients regain function following hospitalization, surgery, illness, or injury Educate patients, caregivers, and family members Collaborate with nurses, PTs, SLPs, physicians, social workers, and other healthcare professionals Complete accurate and timely EMR documentation Strong OTs Can Come From Different Settings You don't necessarily need previous home health experience to be successful here. We'd like to meet Occupational Therapists coming from: Home Health Acute Care Inpatient Rehabilitation Skilled Nursing Transitional Care Outpatient Therapy Orthopedic Rehabilitation Neurological Rehabilitation Senior Living Geriatrics If you've developed strong assessment skills, clinical judgment, patient education abilities, and confidence making treatment decisions , those skills can translate well to home health. What We're Looking For Current Minnesota Occupational Therapist (OTR/L) license Valid driver's license and reliable transportation Strong evaluation and treatment-planning skills Ability to independently manage a territory-based caseload Excellent patient and caregiver communication Strong organization and documentation skills EMR experience preferred Commitment to patient-centered, functional therapy More Independence Doesn't Mean Less Support We want OTs who are confident making clinical decisions, but that doesn't mean you're practicing on an island.
