Job Description
Liberty Health is seeking a Home Care Occupational Therapist to deliver person-centered therapy services to adults in their homes. In this role, you will assess functional abilities, develop individualized treatment plans, and implement evidence-based interventions to improve safety, independence, and quality of life. You will collaborate with an interdisciplinary team, educate patients and caregivers, document outcomes, and adjust plans based on progress.
