Job Description

Liberty Health is seeking a Home Care Occupational Therapist to deliver person-centered therapy services to adults in their homes. In this role, you will assess functional abilities, develop individualized treatment plans, and implement evidence-based interventions to improve safety, independence, and quality of life. You will collaborate with an interdisciplinary team, educate patients and caregivers, document outcomes, and adjust plans based on progress. This position offers the opportunity to support diverse populations, contribute to community health, and drive continuous improvement in home-based care.
