Job Description

Stratford Manor Rehabilitation & Care Center in West Orange, NJ is seeking a compassionate Occupational Therapist to provide resident-centered care in our skilled nursing and rehab setting. In this role, you will evaluate residents, develop individualized treatment plans, and deliver evidence-based interventions to promote safety, independence, and quality of life for both short-term rehab and long-term care residents. You'll collaborate closely with nursing, rehab, and medical teams, educate families, support safe discharge planning, and contribute to a respectful, mission-driven environment focused on strong clinical outcomes and resident dignity. Responsibilities Evaluate residents' functional status and develop individualized occupational therapy treatment plans Provide evidence-based OT interventions to improve ADLs, mobility, strength, and safety Support short-term rehab and long-term care residents in achieving functional goals and discharge plans Collaborate with nursing, rehab, and medical teams to coordinate holistic resident care Educate residents and families on adaptive techniques, equipment use, and home safety Document evaluations, goals, progress notes, and outcomes in the EMR in accordance with regulations Participate in care plan meetings and contribute to quality and performance improvement initiatives Maintain compliance with state, federal, and facility policies and professional standards Required Skills Occupational therapy evaluation and treatment planning ADL retraining and functional mobility Geriatric and post-acute rehabilitation Home safety and discharge planning Interdisciplinary care coordination and documentation Use of adaptive equipment and DMEPatient and family education EMR documentation for SNF/rehab Therapeutic exercise and neuromuscular re-education Regulatory and Medicare/PDPM documentation knowledge
