A fast-growing, Certified Home Health Agency (CHHA) is seeking a Full-Time Occupational Therapist (OT) to provide home care visits to patients throughout Queens. Develop individualized, patient-specific plans of care. Provide skilled Occupational Therapy services in the patient's home. Complete clinical documentation accurately and submit it to the agency in a timely manner. Monitor patient progress and modify the plan of care as appropriate. Promote patient safety, independence, and satisfaction. Communicate effectively with patients, families, and the home care team.
