Job Description

Madison Health Care Nursing and Rehabilitation in Madison, OH is seeking a PRN or Full Time Occupational Therapist (OT) to join our growing In-House therapy team. This is an excellent opportunity for a compassionate and motivated clinician looking to make a meaningful impact in a collaborative, skilled nursing and rehabilitation setting. Madison Healthcare is a 125-bed skilled nursing facility in Madison, Ohio. We strive to make you feel at home in our private rehabilitation suites as you recover and work toward your wellness goals. We provide expert skilled nursing services to address medical complexities caused by illness, injury, surgery and hospitalization. Our welcoming facility is just off the coast of Lake Erie and offers convenient access to downtown Madison Village and Lake Health Emergency Rooms. Clinical services include long-term care, skilled nursing services, memory care and behavioral support. Our facility has seven private suites and semi-private, triple and quad rooms.
Essential Duties & Responsibilities:
Plan treatment to residents and patients with temporary or long-term disabilities to relieve pain, restore or improve function, and promote healing Assure all treatment is delivered in accordance with an established plan of care Provide clinical support and instruct patients, families, and caregivers Monitor patient response to treatment intervention Complete required forms and documentation in accordance with company policy and state/federal regulations Consulting with patients to learn about their physical condition. Assessing and interpreting patient evaluations and test results. Developing treatment plans using a variety of treatment techniques. Creating personalized fitness-oriented health care programs for patients. Administering medically prescribed physical therapy treatments to relieve pain and improve mobility. Advising patients on exercise techniques. Advising patients and their families about in-home treatment options. Providing educational information about injury prevention, ergonomics and ways to promote physical health. Consulting and collaborating with other healthcare professionals. Documenting patient care history. Complying with rules, regulations, and procedures.
Qualifications:
Associate degree from an accredited Occupational Therapy program. Current OT license in the state of employment. CPR certification required. Previous experience in skilled nursing, rehab, or long-term care preferred. Strong understanding of rehabilitation principles, restorative care, and geriatric populations. Knowledge of Medicare Part A, Part B, and managed care preferred. Excellent communication and teamwork skills. Apply Today All inquiries will be held confidential
