Job Description

Job Summary Provides Occupational therapy services to home care patients in accordance with the Plan of Care. Services are provided under the direction of the attending physician with participation of the family. The therapist will manage and re-evaluate the appropriateness of the maintenance program. Responsibilities and Duties Organizes and performs physician ordered therapy program for patients involving self-care training, home management and cognitive retraining. Modifies treatment programs as indicated by patient condition and instructs caregivers of home Occupational Therapy programs. Performs specific age-related therapy. Performs direct treatment, initial.interim evaluations, physicians Plan of Care and discharge visits/summary. OT will evaluate patient's home occupational therapy needs to include but not limited to: functional physical limitations rehab potential, environmental safety and caregiver involvement. Confers with physician as indicated with findings, recommendations for changes and reports patient responses to treatment. Performs home safety/fall risk assessments and documents appropriately Plans, prepares and carries out individually-designed programs of physical treatment to maintain, improve or restore physical functioning,l alleviate pain and prevent physical dysfunction in patients. Performs and documents an initial assessment, evaluating the data to identify problems and determine a plan of care/intervention.
Qualifications and Skills Licenses:
Current license to practice as an Occupational Therapist in the Commonwealth of Pennsylvania CPR Certification Registration by the American Occupational Therapy Association Valid driver's license
Experience:
Two years prior Occupational Therapy experience required One year prior experience providing therapy in the home setting preferred
Knowledge and Abilities:
Ability to plan, implement and evaluate the effectiveness of the Occupational Therapy Program Ability to document appropriately in the clinical record Intermediate computer skills

Full-time Pay:
$25.00 - $35.00 per hour
