Job Summary:
Uniontown Nursing and Rehab is seeking a dedicated and compassionate Occupational Therapist (OT) to join our skilled nursing and rehabilitation team in Uniontown, Pennsylvania. Role:
As an Occupational Therapist (OT) at Uniontown Nursing and Rehab, you will be responsible for designing and implementing individualized occupational therapy plans to improve the functional abilities of our residents. You will work closely with our interdisciplinary team to identify and address the rehabilitation needs of our residents, and provide hands-on therapy to help them achieve their goals. Our state-of-the-art therapy department is equipped with the latest technology and equipment, and we are committed to staying at the forefront of rehabilitation best practices.
Responsibilities:
Develop and implement individualized occupational therapy plans to meet the unique needs of each resident Conduct thorough evaluations to assess resident functional abilities and identify areas for improvement Provide hands-on therapy sessions to help residents regain and improve their motor skills, cognitive function, and daily living activities Collaborate with our interdisciplinary team to ensure seamless care coordination and communication Document resident progress and adjust therapy plans as needed Maintain accurate and thorough records of resident care and treatment
