Spanish Meadows Nursing & Rehabilitation is seeking a compassionate, dependable, and organized Licensed Occupational Therapist (OT) to join our Rehabilitation Therapy Department. The Occupational Therapist will provide skilled occupational therapy services to our residents and outpatient patients , including evaluations, re-evaluations, treatment planning, progress assessments, required visits, documentation, and other duties that require the involvement of a licensed Occupational Therapist. This position will work closely with our existing Occupational Therapy Assistant (OTA/COTA) team under the supervision of our Director of Rehab to coordinate resident care, ensure timely completion of required documentation, and help maintain an efficient and compliant Occupational Therapy program. The ideal candidate is comfortable working independently while also serving as an active member of an interdisciplinary rehabilitation team. Essential Duties and Responsibilities The Occupational Therapist will: Perform comprehensive Occupational Therapy evaluations and re-evaluations for residents and outpatient patients. Develop individualized plans of care based on each patient's needs, functional abilities, physician orders, and therapy goals. Complete required progress assessments, 10th-visit documentation, recertifications, discharge documentation, and other OT-required clinical documentation . Provide direct Occupational Therapy treatment when appropriate. Review and sign documentation requiring approval or signature by the Occupational Therapist. Coordinate treatment plans and patient care with the facility's OTAs/COTAs . Provide appropriate supervision, direction, and clinical support to

staff. Monitor resident progress and modify treatment plans when clinically indicated. Respond to changes in resident condition and determine whether additional evaluation or modification of the plan of care is necessary. Ensure evaluations and other required OT services are completed in a timely manner. Communicate with nursing staff, physicians, rehabilitation staff, residents, families, and other members of the interdisciplinary team as appropriate. Participate in care planning and interdisciplinary communication when needed. Maintain accurate, timely, and complete clinical documentation. Enter and verify hours worked and treatment activity in the facility's Therapy System for productivity tracking. Monitor personal productivity and work with Therapy leadership to appropriately manage the daily caseload. Ensure that treatment time, documentation, evaluations, and other therapy activities are accurately recorded in the Therapy System. Follow facility policies, documentation requirements, resident rights, HIPAA requirements, infection-control practices, and applicable federal and state regulations. Assist the Therapy Department in maintaining appropriate clinical coverage based on census and therapy caseload. Perform other Occupational Therapy responsibilities within the scope of licensure as needed.

Qualifications Applicants must:

Be a graduate of an accredited Occupational Therapy program. Hold a current and unrestricted Texas Occupational Therapist license , or be eligible to obtain Texas licensure. Meet all applicable requirements to practice Occupational Therapy in Texas. Have strong clinical assessment and documentation skills. Be comfortable working with geriatric and rehabilitation patients. Be able to communicate effectively with residents, families, nursing staff, therapy staff, and other members of the interdisciplinary team. Be comfortable utilizing electronic documentation and therapy management systems. Be dependable, organized, and able to manage evaluations, documentation, treatment responsibilities, and deadlines appropriately. Preferred Qualifications Previous skilled nursing facility, long-term care, rehabilitation, or outpatient therapy experience. Experience supervising or coordinating with OTAs/COTAs. Familiarity with therapy productivity systems and electronic documentation. Experience working with geriatric residents and patients with complex rehabilitation needs. New graduates who meet licensure requirements and demonstrate strong communication, organization, and clinical skills are also encouraged to apply.
