Responsibilities include:
Evaluating residents and identifying functional needs Developing and implementing individualized treatment plans Addressing ADLs, IADLs, and functional independence Monitoring resident progress and adjusting treatment plans as needed Recommending adaptive equipment and safety strategies Educating residents, families, and caregivers Collaborating with nursing, physicians, and the interdisciplinary care team Completing timely and accurate documentation Participating in care planning and team collaboration What We're Looking For Active Texas Occupational Therapist (OT) license In good standing with applicable licensing boards and regulatory agencies LTC/SNF or geriatric experience preferred, but not required Strong clinical, communication, and interpersonal skills Net Health experience is a plus Passion for helping residents maintain independence and quality of life At FPACP, we take pride in our culture and our ROCKIN recognition program, ROCKStar (Recognizing Outstanding Care and Kindness) .
