In this flexible role, you will provide specialized occupational therapy services to our skilled nursing facilities. Your expertise will help individuals of the geriatric population to achieve their maximum functional independence. This position offers the opportunity to make a meaningful impact through evidence-based practice, innovative treatment plans, and compassionate patient care. Duties Conduct comprehensive occupational therapy assessments to evaluate patients' physical, cognitive, and sensory abilities. Develop and implement personalized treatment plans aimed at improving daily functioning, motor skills, and adaptive techniques. Assist patients with Activities of Daily Living (ADLs), including dressing, grooming, feeding, and mobility tasks. Support discharge planning by coordinating care strategies with multidisciplinary teams and ensuring smooth transitions across care settings. Document clinical progress accurately using electronic health records (EHR) systems while maintaining thorough medical documentation in compliance with healthcare standards. Collaborate effectively with speech therapists, physical therapists, case managers, educators, and families to optimize patient outcomes. Provide education and training on occupational therapy techniques to patients, caregivers as needed. Participate in clinical interprofessional collaboration to ensure holistic patient care and evidence-based practice implementation. Experience Proven experience working with individuals across the lifespan for geriatrics and adult populations in skilled nursing facilities. Prior experience assisting with ADLs and developing treatment plans tailored to diverse needs. Familiarity with acute care environments like ICU or medical-surgical units is highly desirable. Knowledge of medical terminology and physiology relevant to occupational therapy practices is essential for accurate documentation and effective treatment planning. Skilled in implementing occupational therapy programs that support clients' physical rehabilitation goals or occupational health needs. Ability to communicate clearly with patients of all ages and collaborate seamlessly within interdisciplinary teams for comprehensive care delivery.
