Duties Conduct comprehensive evaluations of patients' physical and cognitive abilities using standardized assessments and clinical observations Develop personalized care plans that address individual needs, goals, and medical conditions Assist with activities of daily living (ADLs) such as dressing, grooming, feeding, and mobility to promote independence Collaborate with multidisciplinary teams including physicians, speech therapists, physical therapists, and social workers to coordinate holistic care Provide therapy sessions focused on improving motor skills, sensory processing, and functional performance in various settings such as inpatient units, outpatient clinics, schools or home health visits Support discharge planning by preparing patients for transition back to home or community living environments Educate patients and caregivers on adaptive techniques and equipment to enhance safety and independence Maintain detailed documentation of patient progress, treatment plans, and case management records in compliance with healthcare regulations Requirements Bachelor's or Master's degree in Occupational Therapy from an accredited program; current licensure or registration as an Occupational Therapist is required Proven experience working with diverse populations including pediatrics, geriatrics, individuals with disabilities or neurological conditions Knowledge of physiology, anatomy, medical terminology and therapy techniques relevant to occupational therapy practice Experience in acute care settings such as ICU or hospital units is highly desirable Familiarity with post-acute care environments including skilled nursing facilities and home health services Ability to assist with ADLs and develop effective care plans tailored to individual needs Strong case management skills with the ability to coordinate multidisciplinary interventions effectively Excellent communication skills for working with patients of all ages and backgrounds in outpatient clinics or school settings Demonstrated understanding of early intervention strategies and occupational health principles Join us in delivering compassionate care that transforms lives!
