Job Description

CenterWell Home Health is seeking a per diem Occupational Therapist to deliver high-quality, in-home therapy for seniors and adults with chronic conditions. In this role, you will perform comprehensive evaluations, develop individualized care plans, and provide evidence-based interventions to improve safety, independence, and daily function at home. You'll collaborate closely with an interdisciplinary team, educate patients and caregivers, and document care in our electronic systems while supporting a compassionate, patient-first culture. Responsibilities Provide in-home occupational therapy evaluations for adult and senior patients Develop and implement individualized treatment plans and functional goals Deliver therapeutic interventions to improve ADLs, safety, and independence Educate patients and caregivers on home exercise programs and adaptive techniques Assess home environment for safety risks and recommend modifications or equipment Collaborate with nurses, therapists, and physicians on coordinated care plans Document assessments, visits, and outcomes in the electronic health record accurately and timely Monitor patient progress and adjust interventions and goals as needed Communicate changes in condition and care needs to the interdisciplinary team Maintain professional, ethical, and regulatory standards in all aspects of care Required Skills Home health occupational therapy Patient assessment and evaluation Care planning and goal setting
