Job Description

Deliver essential occupational therapy services directly to patients in their homes, empowering individuals to maximize their independence and enhance their quality of life. This contract position offers the opportunity to make a meaningful difference while collaborating within a skilled interdisciplinary healthcare team.
Schedule:
Monday Friday, 8:00 AM 5:00 PM Full-time, 13-week contract
Qualifications:
Active and unrestricted Occupational Therapist (OT) license in Washington State Minimum 1 year of recent home health occupational therapy experience Hands-on proficiency with OASIS documentation (required) Familiarity with Home Care Home Base (HCHB) is preferred Solid clinical assessment and functional training abilities Strong organizational, time management, and communication skills Comfortable managing an independent, field-based caseload Reliable transportation, valid drivers license, and auto insurance
Key Responsibilities:
Perform comprehensive occupational therapy evaluations in patient homes, utilizing OASIS as needed Develop and implement individualized care plans focused on
ADL/IADL
improvement, mobility, cognition, and safety Provide skilled interventions including therapeutic activities, self-care and energy conservation training, and adaptive equipment instruction Recommend and facilitate acquisition of durable medical equipment (DME) as appropriate Monitor and document patient progress, updating care plans and communicating with care team members and physicians Educate patients and caregivers on strategies for safety, independence, and fall prevention Collaborate effectively with nurses, physical therapists, social workers, and other healthcare professionals Participate in interdisciplinary meetings and ongoing professional development
