Role:

Provide occupational therapy services to patients in our skilled nursing and rehabilitation center, including assessments, interventions, and evaluations Develop and implement individualized treatment plans to help patients achieve their goals and improve their functional abilities Collaborate with other members of the healthcare team to ensure comprehensive care and effective communication Document patient progress and outcomes in accordance with regulatory requirements and professional standards

Responsibilities:

Conduct initial and ongoing assessments of patients' physical, emotional, and cognitive abilities Design and implement customized occupational therapy programs to address patients' specific needs and goals Provide education and training to patients, families, and caregivers on functional activities, adaptive equipment, and other rehabilitation techniques Maintain accurate and detailed records of patient treatment, progress, and outcomes Participate in interdisciplinary team meetings to discuss patient care and treatment plans

Requirements:

Bachelor's or Master's degree in Occupational Therapy from an accredited program Current OT licensure in the state of Wisconsin Strong communication and interpersonal skills Ability to work effectively in a fast-paced, dynamic environment Strong analytical and problem-solving skills
