About Us Reach To Win Youth Home and Family Services is a Short-Term Residential Therapeutic Program (STRTP) that provides residential care and mental health services for foster girls ages 12-17. We are committed to supporting our residents through individualized treatment, family engagement, and community collaboration to ensure safety, permanency, and long-term success. Position Overview We are seeking a dedicated Mental Health Rehabilitation Specialist (MHRS) to join our team. The MHRS plays a key role in the treatment of residents and their families by conducting assessments, developing treatment plans, and providing targeted interventions. This position collaborates closely with the Child and Family Team, school representatives, outside agencies, and licensed clinicians to support behavioral stability, permanency, and well-being. This role provides valuable opportunities to work alongside licensed and registered clinicians (LCSW, ACSW, AMFT) in a collaborative environment that promotes professional learning and growth. Minimum Qualifications Education Bachelor's degree in psychology, social work, child development, counseling, or a related field from an accredited institution (required). Experience Minimum of four (4) years of direct service experience with severely emotionally disturbed children in an inpatient or outpatient healthcare setting. Internship or practicum experience may be substituted. License/Certification Valid California Driver's License and a clean driving record. Specialized Skills Strong verbal and written communication skills with the ability to interact effectively with youth, families, staff, and community professionals. Knowledge of psychiatric illnesses, treatment interventions, and behavioral health principles. Ability to visually and audibly assess the mental and psychosocial status of residents. Understanding of community resources and county agency systems.

Physical Requirements:

Ability to observe and respond to resident behaviors and needs. Ability to sit, complete documentation, and communicate by phone for extended periods (up to 1 hour at a time). Potential Job Hazards Working with assaultive or aggressive youth. Exposure to blood and body fluids (Category III). Operation of company vehicle. Safety Requirements Knowledge and application of crisis intervention and response strategies. Consistent use of universal precautions. Maintenance of a safe driving record and participation in vehicle safety orientation. Essential Duties & Responsibilities Under the supervision of the Head of Service, the MHRS will: Conduct evaluations and re-evaluations of residents to assess psychosocial, medical, educational, and environmental needs. Review medical, psychosocial, school, and other relevant records. Collaborate with residents, families, treatment staff, placement staff, consultants, and community agencies to identify needs and coordinate services. Assist in the development of individualized Mental Health Treatment Plans and Crisis/Safety Plans, linking needs and strengths to targeted goals. Provide crisis intervention services by phone or in person to support residents during emergencies. Deliver targeted interventions to improve behavioral outcomes and make recommendations for services to the Head of Service. Monitor, support, and assist residents in achieving treatment plan goals, including access to medical, dental, psychiatric, educational, and social services. Connect residents and families with community resources such as social services, welfare, health departments, and other agencies. Maintain consistent communication with families, guardians, and support networks to strengthen relationships and ensure continuity of care. Actively participate in Child and Family Team (CFT) meetings, advocate for resident needs, and follow up on action items. Document all services, interventions, and outcomes in accordance with program and regulatory standards. Perform other duties as assigned by the Head of Service or designee.

$30.00 - $32.00 per hour
