Role Overview Physical Therapists (PT) & Physical Therapist Assistants (PTA) Perform or support patient evaluations focused on mobility and physical function Create and carry out treatment plans aimed at improving strength and independence Deliver therapeutic exercises, gait training, and patient mobility education Guide patients and caregivers on safety and home exercise programs Ensure timely and accurate documentation while coordinating with the care team Occupational Therapists (OT) & Certified Occupational Therapy Assistants (COTA) Assess or assist in assessing patients' functional capabilities Develop or implement care plans to improve daily living skills and independence Provide training on adaptive techniques and assistive equipment Monitor patient progress and update treatment plans as needed Maintain compliant and thorough documentation Speech Therapists (ST) Assess and treat speech, language, cognitive, and swallowing disorders Design and implement individualized therapy programs Educate patients and families on communication techniques and exercises Track outcomes and maintain detailed clinical records Licensed Clinical Social Workers (LCSW) Conduct psychosocial evaluations and provide counseling services Develop care plans addressing emotional, social, and resource-related needs Help patients access community services, benefits, and financial assistance Work closely with the healthcare team to ensure comprehensive care Provide crisis support and maintain accurate documentation Qualifications (All Roles) Active California license in good standing (PT, PTA, OT, COTA, SLP, or LCSW) EIN or willingness to obtain one Valid California driver's license and dependable transportation Active auto insurance Current CPR certification Recent physical exam and TB clearance Strong communication, documentation, and organizational abilities Ability to work independently and manage a flexible schedule Prior home health experience is a plus, but not required What We Offer Competitive per-visit compensation Coverage opportunities throughout San Gabriel Valley Supportive and collaborative team culture The opportunity to positively impact patients' quality of life How to Apply Please submit your resume to hr@knrehabinc.com We look forward to connecting with you!
