Service Facilitator / Community Rehabilitation Specialist Comprehensive Community Services (CCS) - Sauk County (25-40 hrs/week) Join a dynamic, team-based program supporting individuals with mental health and/or substance use needs. As a Service Facilitator, you'll collaborate with clients and families to assess needs, develop care plans, and coordinate individualized, goal‑driven services, as well as work closely with other CCS providers using a wraparound team approach. In addition to Service Facilitation, candidates also have the option to serve as a Community Rehabilitation Specialist (CRS). In this role, you'll work one on one with clients to support progress toward identified goals through skill‑building, psychoeducation, and development of daily living skills. CRS providers offer hands‑on, practical support that helps clients increase independence, strengthen coping strategies, and build confidence in navigating their day to day lives.

Key Responsibilities:

Conduct assessments and develop recovery-focused care plans Provide or coordinate services such as skill development, psychoeducation, employment support, and other tasks Facilitate and participate in wraparound services and contribute to service planning and progress evaluation Maintain accurate documentation in electronic health records Travel within the community for client visits and team meetings

Qualifications:

Bachelor's or Master's degree in a human services-related field (e.g., Social Work, Psychology, Counseling, Criminal Justice) Valid driver's license, reliable transportation, and auto insurance Experience with CCS or wraparound case management preferred Ability to work independently in community-based settings and remotely Basic computer proficiency and comfort with electronic documentation
