Solaris HealthCare seeks a Certified Occupational Therapy Assistant to support resident-centered rehab services in our skilled nursing and long-term care community. Working under the direction of an Occupational Therapist, you'll carry out treatment plans, lead therapeutic activities, and help residents improve independence with daily living tasks. You'll collaborate with an interdisciplinary team, document progress, educate residents and families, and uphold high standards of safety, dignity, and compassionate care in a supportive, growth-oriented environment. Responsibilities Implement OT treatment plans under the supervision of an Occupational Therapist. Provide therapeutic activities to improve ADLs, mobility, and functional independence. Monitor, document, and report resident progress using EMR systems. Educate residents and families on exercises, safety, and adaptive strategies. Collaborate with nursing, rehab, and care teams on individualized care plans. Ensure adherence to infection control, safety, and regulatory standards. Assist with resident assessments and recommend therapy adjustments. Support a resident-centered, compassionate care environment. Participate in team meetings, in-services, and quality improvement efforts. Maintain therapy equipment and treatment spaces in safe condition.
