Job Description
Family Home Care Group is seeking a compassionate and dependable Occupational Therapist to join our growing home health team. This role is ideal for a clinician who enjoys working independently, providing meaningful one-on-one care, and helping patients improve safety, independence, function, and quality of life in the comfort of their homes. We are looking for a professional, organized, and patient-focused OT who is comfortable working with adult and geriatric patients in a home health setting. Responsibilities As a Home Health Occupational Therapist, you will: Evaluate patients in their homes and develop individualized occupational therapy plans of care. Assess patients' ability to perform activities of daily living, including bathing, dressing, grooming, toileting, transfers, meal preparation, and other functional tasks. Provide skilled interventions to improve strength, endurance, balance, coordination, mobility, safety awareness, and independence. Recommend adaptive equipment, durable medical equipment, home modifications, and compensatory strategies as appropriate. Educate patients, caregivers, and family members on safety, fall prevention, energy conservation, transfer techniques, and home exercise programs. Coordinate care with nurses, physical therapists, speech therapists, physicians, and other members of the care team. Document evaluations, visits, progress notes, reassessments, care coordination, and discharges accurately and timely in the EMR. Communicate changes in patient condition, progress, barriers, or concerns to the appropriate clinical team members. Follow Medicare, payer, agency, and regulatory documentation standards. Maintain professional, compassionate, and reliable communication with patients and staff. Qualifications Current Occupational Therapist license in the State of Florida. Degree in Occupational Therapy from an accredited program. Current CPR/BLS certification preferred. Prior home health experience preferred, but not required. Experience with adult and geriatric patients preferred. Strong communication, documentation, and time-management skills. Ability to work independently while collaborating with an interdisciplinary team. Reliable transportation, valid driver's license, and current auto insurance. Must be able to pass required background screening. Ideal Candidate The ideal candidate is someone who is: Compassionate, professional, and dependable.
