Occupational Therapist (OT) / Certified Occupational Therapy Assistant (COTA) Homeside Rehab Join Our Growing Home Health Team! Homeside Rehab is seeking compassionate and dedicated Occupational Therapists (OTs) and Certified Occupational Therapy Assistants (COTAs) to provide individualized, evidence-based occupational therapy services to patients in their homes throughout New York City . If you're passionate about helping patients improve functional independence, mobility, self-care, and quality of life while enjoying a flexible schedule and supportive clinical team, we'd love to hear from you! Responsibilities Perform comprehensive occupational therapy evaluations (OTs) and develop individualized treatment plans based on each patient's needs and goals. Provide evidence-based occupational therapy interventions to improve fine motor skills, upper extremity function, activities of daily living (ADLs), sensory processing, coordination, and functional independence. Deliver skilled therapy services in patients' homes while promoting improved function and quality of life. Complete accurate, timely, and compliant electronic documentation. Collaborate with physicians, caregivers, and the interdisciplinary healthcare team to coordinate patient care. Educate patients and caregivers on home exercise programs, adaptive equipment, and therapeutic strategies. Communicate effectively with office staff regarding scheduling, patient updates, and clinical needs. Qualifications Active New York State Occupational Therapist (OT) or Certified Occupational Therapy Assistant (COTA) License (Required) Minimum 1 year of experience in Home Health, Outpatient, Early Intervention, or Skilled Nursing Facility (SNF) preferred Excellent clinical assessment and communication skills Ability to work independently in a home health setting Basic computer proficiency for electronic documentation Valid New York State driver's license and reliable transportation

$75.00-$100.00 per hour Schedule Choose your own hours Monday through Friday Day and evening shifts available Weekend availability optional Service Areas We are actively hiring throughout: Bronx (10462, 10473, 10451 and surrounding areas) Brooklyn (11211, 11229, 11238, 11249, and surrounding areas) Queens (11355 and surrounding areas) Staten Island (10314 and surrounding areas) Cases are assigned based on your preferred coverage area.
