Occupational Therapist (OT) / Certified Occupational Therapy Assistant (COTA) Homeside Rehab Join Our Growing Home Health Team! Homeside Rehab is seeking compassionate and dedicated Occupational Therapists (OTs) and Certified Occupational Therapy Assistants (COTAs) to provide individualized, evidence-based occupational therapy services to patients in their homes throughout New York City . Responsibilities Perform comprehensive occupational therapy evaluations (OTs) and develop individualized treatment plans based on each patient's needs and goals. Provide evidence-based occupational therapy interventions to improve fine motor skills, upper extremity function, activities of daily living (ADLs), sensory processing, coordination, and functional independence. Deliver skilled therapy services in patients' homes while promoting improved function and quality of life. Complete accurate, timely, and compliant electronic documentation. Collaborate with physicians, caregivers, and the interdisciplinary healthcare team to coordinate patient care. Educate patients and caregivers on home exercise programs, adaptive equipment, and therapeutic strategies. Communicate effectively with office staff regarding scheduling, patient updates, and clinical needs. Qualifications Active New York State Occupational Therapist (OT) or Certified Occupational Therapy Assistant (COTA) License (Required) Minimum 1 year of experience in Home Health, Outpatient, Early Intervention, or Skilled Nursing Facility (SNF) preferred Excellent clinical assessment and communication skills Ability to work independently in a home health setting Basic computer proficiency for electronic documentation Valid New York State driver's license and reliable transportation
