Job Duties and Responsibilities:
Serve a caseload of approximately 25 patients Provide Cal AIM Enhanced Care Management and Community Support Services to all assigned patients Deliver services to all patients on a weekly basis Develop patient's individualized Care Plan. Link members to local, county and state services/resources, including Orange County Medi-Cal Assist in entering patients into HMIS and CES and update records as needed Build rapport with members by communicating personal experiences with a goal of increasing the likelihood of positive behavior changes Coach members to create and implement strategies that minimize risks associated with the identified common health conditions and behaviors Connect members to appropriate programs to address barriers to care and to enhance compliance Follow up with members and serve as a member advocate, delivering their medications and other supplies as needed Coordinate and/or accompany members to medical and social services appointments Introduce tools to promote self-management & self-efficacy Schedule encounters based on member's acuity level using an electronic health record system Work under the supervision of the Care Management Team Supervisor to ensure proper escalation of cases and coordination of care. Assist in providing coverage for other team members as needed Actively participate in Cal AIM Liaison meetings Actively support the achievement of CalOptima Health's vision and goals for its Street Medicine Program as defined in the Street Medicine Operations Manual Engage with providers and patients to create a trusting environment Complete all required documentation in multiple computer systems including care plans and progress notes within 24 business hours of the service occurring. Maintain a good working relationship with all staff on the team. Attend and actively participate in all CalOptima Health led meetings Participate in case conferences Close encounter notes in a timely manner and per Cal Optima and any other governing organization standards Other duties as assigned
Qualifications and Experience:
G.E.D./High School, required plus 3+ years of work experience as defined below Bachelor's degree preferred, plus 1-2 years of work experience as defined below Work Experience to include direct work experience, lived experience, training or volunteer work with patients experiencing homelessness, as well as those with active mental health and substance use conditions A comparable combination of education/experience and/or training will be considered equivalent to the education listed above Experience providing direct services to unhoused individuals, preferred HMIS/ Coordinated Entry System experience, preferred Experience working with community-based organizations a plus Adaptable and open to change and problem solving Excellent customer service skills required Cultural competence and sensitivity required, multilingual skills a plus, especially Spanish or Vietnamese Proof of a valid driver's license in good standing Have access to a vehicle.
Work Schedule:
The typical work schedule is 40 hours per week; however, shifts may vary based on clinic operations. Occasional overtime may be required to meet patient or business needs. CLCHC reserves the right to adjust work schedules or assign employees to different locations as necessary to support organizational needs.
