Summary We are seeking a licensed, experienced Nurse Care Navigator to help support our existing team in Hartford County. This is part-time, per diem work. Hours fluctuate and can largely be set by the Nurse Care Navigator. Candidates must have at least 10 years' experience working with older adults and possess applicable certifications/licenses. They must be able to assess the needs of older adults and provide thoughtful care planning advice and strategies.
