About us Sage Solutions is dedicated to supporting the holistic well-being of older adults across Connecticut. Our team of Care Navigators and other specialists have the knowledge, skills, and passion to improve the health and quality of life for our clients. We offer a supportive, respectful work environment with a highly flexible schedule. Summary We are seeking a licensed, experienced Nurse Care Navigator to help support our existing team in Hartford County. This is part-time, per diem work. Hours fluctuate and can largely be set by the Nurse Care Navigator. Candidates must have at least 10 years' experience working with older adults and possess applicable certifications/licenses. They must be able to assess the needs of older adults and provide thoughtful care planning advice and strategies. Strong advocacy skills and the ability to work within family systems are necessary, as is upholding our mission to improve the quality of life of all clients. Direct experience with older adults is
REQUIRED
; experience with individuals with memory impairment is a plus. Patience, flexibility, compassion, organization, collaboration, and timeliness are all keys to performing this work.
Required Qualifications and Experience Education:
RN required
Experience:
Must have a minimum of 10 years of experience in health care or social services or related field, plus experience working with older adults.
Skills:
Knowledge and understanding of social, health, and economic factors that contribute to health and wellbeing outcomes; knowledge and skill in interviewing and assessment (social and health) techniques. Knowledge of community resources available to individuals; an ability to mobilize resources into a coordinated and comprehensive plan of care. Strong written, verbal and computer skills required. Ability to maintain confidentiality of resident, company, and staff information.

The hours and schedule for this position will vary. The schedule can largely be set by the Nurse Care Navigator.
