The Peer Support Navigator provides outreach to Woodley House clients. Assist clients with basic case management services as well as education and support around recovery goals; problem solving, and leading peer support groups. The Peer Support Navigator provides consistent presence for the residents as well as highly individualized recovery services while promoting consumer self-determination and decision making.
ESSENTIAL FUNCTIONS
Build strong rapport with assigned clients. Advocates with and for the clients to access resources needed to support recovery. Provides counseling to identify and support personal recovery goals while building resiliency. Facilitates rehabilitation, psycho-educational, and recovery support groups by providing expertise and consultation from a client perspective. Encourages and supports clients in building their community and social networks which may include social, recreational, spiritual, educational, or vocational linkages. Supports consumers in developing parenting, life, and decision-making skills. Participates in training relevant to position and to the maintenance of certification. Completes encounter notes from client sessions within 24 hours and maintain EHR documentation for good clinical care and accurate billing. One hundred percent compliance is expected. Consult with clinicians and providers as needed to link clients to appropriate medical, dental, and behavioral health services. 10. Performs other duties as assigned.
ADDITIONAL RESPONSIBILITIES
1. Fully cooperates with the organization's compliance and quality improvement program. 2. Models and reinforces Woodley House mission to provide behavioral health, OUD, STUD, and Other SUD support. 3. Provide residential services, and primary health care coordination for marginalized and disenfranchised men living in the District of Columbia, many of whom are coping with challenges including mental illness, addiction, and the aftermath of trauma and abuse. 4. Models and reinforce Woodley House values of quality, innovation, respect, equity, and integrity daily. 5. Reinforces Woodley House commitment to diversity, equity, and inclusion. 6. Protects the privacy of our consumer's protected health information by maintaining compliance with HIPAA and other relevant CC related IT security regulations. 7. Completes and stays current on role specific and organizational wide training. 8. Perform other duties as assigned on an as-needed basis.
KNOWLEDGE, SKILLS, AND ABILITIES
1. Lived experience with substance abuse and/or mental health issues. 2. Demonstrates basic knowledge of steps involved in meaningful recovery. 3. Ability to build rapport with a diverse audience. 4. Ability to maintain professional boundaries. 5. Ability to work in a team environment. 6. Ability to identify and resolve problems in a timely manner even when dealing with emotional topics. 7. Solves practical problems and manages a variety of concrete variables in situations where only limited standardization exists. Ability to interpret a variety of instructions in both written, oral, diagram, or schedule form. 8. Ability to understand complex clinical concepts (engagement, outreach, empathy, etc.) and interventions over time and to incorporate these into clinical practice under supervision of clinical manager. 9. Proficient in MS Office, including Word, Excel, and Outlook 10. Time Management skills The ability to climb stairs and lift 20 lbs.
EDUCATION AND EXPERIENCE
1. HS Diploma/GED, Minimum one year in recovery required 2. 3-5 years in recovery preferred. Previous experience with running support groups and doing basic case management tasks. 3. One year of OUD, STUD, or other SUD experience required 4. Basic Life support (CPR & AED) certification required.
