This position will support the Palliative Care team. This position requires Level II screening through the Florida Care Provider Background Screening Clearinghouse, managed by the Agency for Health Care Administration (AHCA). Responsibilities:
Is a member of a collaborative practice model that includes patients, nurses, hospital care coordinators, physicians, and other practitioners, caregivers, and the community. Is knowledgeable about disease, treatment, social and economic implications for individuals and families as it pertains to their age, stage of psychosocial development and discharge needs. Is pragmatic in approaching End of Life and ethical issues. Utilizes clinical counseling skills when applicable.
Minimum Qualifications:
Required Education:
Masters in Social Work or Mental Health Counseling.
Required Experience:
1 year social services in a community based social services or health care setting.
