Referral Coordinator & Healthcare Navigator Department:

Clinic Manager Position Summary The Referral Coordinator & Healthcare Navigator provides dual support within the clinic by managing routine referrals and delivering advanced care coordination for patients with complex medical and social needs. This role ensures timely access to specialty care while addressing barriers through coordination with community resources and healthcare partners. Responsibilities Referral Coordination (Routine Patients) Send, process and track referrals for specialty care. Ensure completeness of required documentation prior to submission. Coordinate with volunteer specialists and partner organizations. Communicate with patients regarding expectations/instructions & follow up throughout referral process Monitor referral completion, obtain records, and document outcomes in EMR. Escalate patients with barriers for Healthcare Navigation Healthcare Navigation (Complex Patients) Identify patients with medical, social, or financial barriers. Perform needs assessments and develop care coordination plans. Coordinate multi-specialty and community-based services. Address barriers including transportation, medication access, and social needs. Provide ongoing follow-up and serve as primary contact for complex patients. Advocate for timely access to necessary care. Escalate to primary care provider when necessary Community Resource Coordination Maintain knowledge of local healthcare and social service resources. Establish and maintain community partnerships. Connect patients to support services and assistance programs.
Administrative / Quality
Maintain accurate EMR & Excel Spreadsheet documentation for referral tracking. Assist with reporting and quality improvement initiatives. Support clinic workflow and team communication. Cross train on clinical skills to provide backup when needed Qualifications Registered Nurse (RN) or Certified Medical Assistant required Minimum 2 years' experience in care coordination, referrals, or case management Proficient in Excel Spreadsheets and EMR navigation Core Competencies Differentiation of routine vs. complex patient needs Critical thinking and problem-solving Strong organizational and communication skills Knowledge of community resources Patient advocacy and teamwork
Pay:
$25.00 - $31.00 per hour
