Key Responsibilities:

Establish and maintain relationships with healthcare professionals, healthcare facilities, and community organizations to promote Prairie Winds Hospice services and facilitate referrals Provide education and training to healthcare professionals and facilities regarding our services and the benefits of hospice care Conduct needs assessments and identify opportunities to improve the care experience for our patients and their families Collaborate with our interdisciplinary team to develop and implement patient-centered care plans Maintain accurate and detailed records of patient referrals, admissions, and interactions with healthcare professionals and facilities Complete admission assessments and all required documentation for admission

Requirements:

3 years of experience as a Clinical Liaison or in a related field (hospital, healthcare, or social services) RN required Strong communication, interpersonal, and organizational skills Ability to work independently and as part of a team Compassionate and empathetic approach to patient care and family support Valid driver's license and reliable transportation
