Discounts on wide array of services/entertainment nationwide The Recovery Support Navigator is responsible for: Providing quality services and case management to assigned patients in accordance with the Recovery Navigator model. Performing intake assessments on all new patients to the program and completes all associated documentation within required time frames. Working with the patient to develop and implementing an individualized plan with goals and objectives based on identified needs. Assisting patients with scheduling and coordinating medical and behavioral health appointments. Coordinating care and the plan with existing providers including but not limited to behavioral health, primary care and Community Partner.
Qualifications Qualifications:
Bachelor's Degree in human services related field or discipline Or LADC Or has a recognized certification and/or licensure for the target population to work with Or sufficient experience and training in counseling and meets regulatory requirements
