Full Job Description Title:
Patient Navigator Job Summary:
The Patient Navigator functions as a clinical support liaison between patients, providers and care team members throughout Osborn Family Health Center and reports to the Chief Medical Officer/Chief Executive Officer. Also coordinates patient-centered care as a liaison between referring physicians, hospital facilities and all other collaborating health care entities. This position will also work in collaboration with all applicable departments.
Summary of Accountabilities:
Always provides high quality customer service to patients in person and over the telephone. Maintain current demographic information on patients (name change; address; insurance updates). Always maintain strict HIPAA compliance. Assist in the navigation of patients to reduce gaps in care. Track and monitor provider orders for outpatient services to include laboratory, radiology, diagnostic testing, and consultation services. Attach results and/or consult reports for outpatient services including laboratory, radiology, diagnostic testing, and consultation services for provider review. Work to improve patient compliance through telephone calls, written correspondence, and the patient portal. Assist patients, when necessary, with lost or misplaced orders upon request. Notify ordering provider of patient non-compliance. Facilitate the process for follow-up appointments as needed. Complete Referrals and Prior Authorization for Providers and Specialty Care Services. Willingness to engage in training, i.e., webinar, webcast, any on-line opportunities. Make recommendations on how to improve efficiency in departments. Willingness to cooperate with covering of staff members and/or make recommendations to manager. Ensure compliance with all company policies, procedures, and guidelines. May sometimes undertake a variety of special projects and/or administrative duties to assist in the efficiencies of the organization. Communicate concerns/patient issues with CMO/CEO /or Risk/Claims Manager. Assists with any additional duties as needed or as directed by CMO/CEO.
Minimum Degree and Experience Required:
Minimum high school diploma or equivalent. Preferred 1-two years' experience as a Certified Medical Assistant Familiarity with IDC10 and CPT coding preferred
Skill Requirements:
Fluent in Spanish/English Ability to assist with patients who present with language barriers. Translation Video/Translation of Telephone. Ability to be detailed oriented and organized. Ability to communicate to promote effective working relationships with other members of staff Ability to work effectively under pressure Ability to perform diversified duties with a high degree of accuracy.
