Responsible for identifying critical resources for clients, helping them navigate through health care services and systems, and promoting client health.

Principal Accountabilities:

1. Offers medical guidance and acts as a support for group. 2. Guides patients and their families through the treatment process. 3. Navigates patients through the diagnostic evaluation. 4. Educates and supports each patient empowering them to make informed treatment decisions. 5. Remains a support system throughout the patient's treatment. 6. Performs other related duties as assigned.

Required:

1. High School Diploma or equivalent. 2. Experience and knowledge of all aspects of the healthcare system.

Preferred:

Certification as a Certified Patient Navigator (CPN) or Community Health Worker (CHW) preferred or willingness to obtain after hire
