Regional Care Navigator Alzheimer's Disease Caregiver Support Initiative The Research Foundation for SUNY Plattsburgh is pleased to announce the Regional Care Navigator position for the Alzheimer's Disease Caregiver Support Initiative (ADCSI), a full time, in-person position with an office located in Queensbury to focus on caregivers in the Washington and Essex County regions. The goal of this initiative is to develop and sustain an innovative array of support services, improve coordination and access to new and existing community resources and enhance access to health and well-being opportunities to caregivers of individuals with dementia.

Duties:

• Promote the services of the ADCSI.
• Perform new and follow up assessments with caregivers to determine needs.
• Provide individual and family care assessments.
• Complete required daily, weekly, monthly, and quarterly, service documentation.
• Complete initial respite applications and ensure that all paperwork is submitted for processing.
• Complete Fall Prevention assessments identify safety concerns and make referrals as needed.
• Refer caregivers to internal and community resources based upon caregiver needs and their request for services, including Project Life Saver and Mindful Music.
• Complete and maintain required annual documentation for NYS Department of Health.
• Actively participate in staff meetings Project Director and Program Coordinator.
• Attend Long Term Care Counsel meetings.
• Attend Joint enrichment programs within the region.
• Other duties as assigned by the Program Director.

This is an in-office position and travel throughout the region to promote and provide services is required and reimbursed at the IRS rate.

Minimum Qualifications:

The successful applicant will be a self-starter, able to work under general supervision. A Bachelor's degree is required and at least 2 years of relevant experience in service coordination or case management. Reliable transportation and a valid NYS driver's license are required. Mileage will be reimbursed.

Special Qualifications:

Strong people skills, proven problem-solving capabilities, and clear verbal and written communication skills. Displays knowledge on effective responses to individuals under stress and interviewing experience that demonstrates calm and respectful techniques. Formal experience conducting assessments to identify needs, strengths, and goals. Proficiency in tracking client progress, making referrals and maintaining accurate, timely and confidential case documentation. Knowledge regarding community resources and programs. Comfort with office equipment (phones, printers, copiers), common computer programs (excel, google), and basic software to enter and track tasks.

Salary Range:

$45,000 - $50,000 with an excellent fringe benefits package to be included.
