• We are a synergized, team-oriented agency Position Summary TheUnite Us Screening & Care Navigator is responsible for identifying individuals who may benefit from health, behavioral health, substance use, social service, and community-based resources and connecting them with appropriate services.

This position serves as a bridge between clients and community providers by utilizing the Unite Us platform to screen for needs, make referrals, coordinate services, and monitor referral outcomes. The Navigator works closely with clients, clinical staff, community organizations, healthcare providers, social service agencies, and other referral partners to reduce barriers to care and improve access to services. Essential Responsibilities Conduct screenings to identify client needs, including behavioral health, substance use, housing, transportation, food insecurity, employment, healthcare, financial, and other social determinants of health. Utilize the Unite Us platform to identify appropriate community resources and make electronic referrals. Educate clients about available services and assist them in accessing appropriate resources. Follow up with clients and referral agencies to determine whether services were successfully accessed. Track referrals, outcomes, and unresolved needs through Unite Us and agency documentation systems. Identify barriers preventing clients from engaging in services and work collaboratively to develop solutions. Provide warm handoffs and care navigation when clients require additional assistance connecting with providers. Maintain communication with community partners and develop positive working relationships with referral agencies. Coordinate with counselors, case managers, medical providers, peer recovery staff, probation, courts, hospitals, and other professionals when appropriate and authorized. Assist clients with navigating healthcare, behavioral health, substance use treatment, and community support systems. Maintain accurate, timely, and confidential documentation in accordance with agency, HIPAA, and regulatory requirements. Participate in team meetings, case conferences, trainings, and community outreach activities. Identify gaps in available community services and communicate those needs to agency leadership. Support outreach and engagement efforts designed to increase access to treatment and supportive services. Maintain professional boundaries and follow all agency policies and procedures. Qualifications High school diploma or equivalent required Strong communication, organizational, problem-solving, and interpersonal skills. Ability to work independently and as part of a multidisciplinary team. Ability to maintain confidentiality and work with sensitive client information. Basic computer skills and ability to learn and utilize electronic referral and documentation platforms. Must demonstrate cultural humility, professionalism, compassion, and a nonjudgmental approach to working with clients.

Pay:

$21.00 - $23.00 per hour Expected hours: 40.0 per week
