Job Description

The Department of Social Work at Jamaica Hospital Medical Center provides a comprehensive continuum of inpatient, outpatient, and community-based behavioral health services. We are seeking a compassionate and dedicated Peer Bridger/Advocate to join our team. The Peer Bridger program utilizes a recovery-oriented approach to provide peer support to individuals experiencing mental health challenges. Through trust-based relationships, advocacy, and mentorship, the Peer Bridger helps foster hope, promote empowerment, and support successful reintegration into the community following hospitalization. This role serves as a vital bridge between inpatient treatment and community-based recovery, ensuring individuals receive meaningful support during and after discharge. Key Responsibilities Provide peer support during the triage process, helping patients understand procedures, reduce anxiety, and facilitate a smooth transition into care. Partner with patients and the treatment team to advocate for appropriate discharge planning and recovery-focused services. Support patients throughout the discharge process to ensure successful transition into the community. Assist patients in identifying and accessing social supports, community resources, and recovery services. Conduct post-discharge follow-up through telephone outreach and home visits for approximately 30 days or longer based on individual needs. Encourage treatment engagement and retention while promoting recovery and self-advocacy. Provide timely communication and reporting to supervisors regarding situations requiring urgent intervention. Maintain accurate and timely documentation of services provided. Collaborate effectively with interdisciplinary treatment teams to support patient goals and continuity of care. Qualifications Required High School Diploma or GED. Demonstrated knowledge of the New York State public mental health and substance use treatment systems. Knowledge of advocacy, self-help, recovery, and empowerment programs for individuals receiving behavioral health services. Ability to inspire hope and promote recovery for individuals living with mental health challenges. Strong written and verbal communication skills. Proficiency with Microsoft Office Suite and internet-based applications. Ability to complete documentation accurately and in a timely manner. Preferred Lived experience with the mental health system and personal recovery journey. Previous experience providing peer support or advocacy services in a behavioral health setting. Certified Recovery Peer Advocate (CRPA) or willingness to obtain certification. Bilingual candidates are encouraged to apply.
