Role:

Hosptial Liaison will serve as the primary point of contact between the community and healthcare providers, ensuring prompt and accurate communication regarding resident care needs and treatment plans. Collaborate with healthcare providers to coordinate medical appointments, medication management, and other healthcare-related services. Develop and maintain positive relationships with healthcare providers, ensuring consistent communication and cooperation to ensure the best possible outcomes for our residents. Hospital Liaison will also assist with the coordination of resident discharges, transfers, and admissions, ensuring a smooth transition for our residents.

Responsibilities:

Hospital Liaison will maintain accurate and timely records of resident care, treatment plans, and communication with healthcare providers. Coordinate and facilitate resident care conferences and meetings with healthcare providers. Participate in quality improvement initiatives to ensure the highest level of care for our residents. Hospital Liaison will also provide educational support and resources to residents and their families, empowering them to take an active role in their healthcare.
