This position provides care coordination and case management services for Medi-Cal members through Enhanced Care Management (ECM) and Community Supports (CS) programs. Conduct outreach and engagement with eligible members. Oversee the delivery of Enhanced Care Management and Community Supports services. Complete assessments, develop care plans, and support successful care plan implementation. Complete referrals and connect members to appropriate services and resources. Provide services in locations that are convenient and accessible to members, including community settings, healthcare settings, and member homes in accordance with Managed Care Plan (MCP) guidelines. Connect members with transportation, community supports, and other social service resources. Advocate for members with healthcare professionals and service providers. Utilize motivational interviewing, trauma-informed care, and harm-reduction approaches. Coordinate discharge planning with hospital staff and medical providers. Accompany members to medical appointments when appropriate and in accordance with MCP guidelines. Monitor treatment and medication adherence. Provide health education, wellness support, and self-management training. Training Provided Lead Care Managers receive training in: Enhanced Care Management (ECM) and Community Supports (CS) Data-sharing methods and processes Member rights and responsibilities Clinical protocols and evidence-based practices Populations of Focus and cultural awareness Patient-centered care practices Health equity and community-based service delivery
