Job Summary The Hispanic Heritage Cultural Institute (HHCI) is hiring two Social Care Navigators to support Medicaid members and community members through New York's 1115 Waiver Social Care Network (SCN) program, in partnership with WNY Integrated Care. Reporting to the Program Director, this part-time, hybrid role screens community members for social determinants of health, connects individuals and families to the community partners that best meet their needs, and follows up to ensure referrals are successfully resolved. This is a hands-on, community-facing, bilingual role ideal for someone who combines strong organizational skills with genuine care for the people HHCI serves. These positions start part-time, with the possibility of becoming full-time as the program and funding grow. Our mission is to celebrate Hispanic arts, culture, and heritage, nurturing a spirit of community and intergenerational learning while contributing to the growth and vibrancy of Western New York. Responsibilities Provide empathetic and culturally competent screening for social determinants of health to community members by phone, virtually, and/or in person. Process screenings and referral intake daily, including calls with potential program participants, documentation, and insurance/eligibility verification. Receive, prioritize, monitor, and triage screenings and referrals that require service connections across community-based organizations. Manage incoming referrals to ensure successful, timely connections between community members and the community partner that best meets their needs. Document progress and action taken for each referral and every interaction in the WNY Integrated Care IT platform, as required. Conduct in-home assessments with community members. Maintain effective communication with community members and/or referring organizations to ensure acceptance and resolution of referrals by the receiving organization, or to redirect a referral to another provider if necessary. Handle community members' questions and concerns with excellent customer service, empathy, and patience. Attend the WNY Integrated Care Screening/Navigation Workgroup. Demonstrate excellent customer service by phone, in person, via email, and in all interactions. Work within WNY Integrated Care social care technology and transportation platforms. Look for opportunities to enhance and simplify the participant experience. Abide by policies and procedures that support federal, state, and local HIPAA and Fraud, Waste, and Abuse regulations. In compliance with federal law, all persons hired will be required to verify their identity and eligibility to work in the United States. Qualifications, Skills & Abilities Experience providing care coordination, care management, or community-based client services; or Community Health Worker (CHW) certification. Successful experience working in community, healthcare, public health, nonprofit, or similar operations. Bilingual fluency in English and Spanish required. Capacity to inspire and motivate others. Extremely detail-oriented and capable of multitasking. Proficient computer skills and a willingness to learn additional software applications. Experience with HIPAA regulations preferred. Ability to relate effectively to diverse groups of people from all social and economic segments of the community, with a track record of building authentic, constructive relationships.
Pay:
$22.00 - $23.00 per hour Expected hours: 15.0 - 20.0 per week
