The role provides navigation, education, HIV/HCV testing, peer recovery support, appointment assistance, and encounter documentation in the electronic health record and TN-WITS. This is a fully in-person position primarily based on the Syringe Services Program mobile unit, with clinic and community-outreach responsibilities. The Peer Navigator/Healthcare Navigator provides patient education, testing support, care coordination, and recovery-focused navigation for individuals receiving services through the clinic, mobile SSP unit, and community outreach settings. This position supports HIV, HCV, STI, and PrEP-related services; assists patients with insurance coverage, patient-assistance programs, appointments, referrals, and follow-up care; and helps link patients to infectious disease care, substance use disorder treatment, medication for opioid use disorder, mental health services, and other health and social-service supports. The role requires accurate documentation in the electronic health record, TN-WITS, and program logs, as well as participation in reporting, meetings, supervision, training, and related program activities.
