Community Health Navigator, Blueprint for
Health Position Summary:
The Community Health Navigator works in collaboration with all members of the health care team. Focuses primarily on supporting individuals recently served in the SVMC Emergency Department. Utilizes appropriate screening tools to identify needs and link patient to services. Develops care initiatives in partnership with team members. Accepts referrals for the navigation of individuals in recovery to services, resources, social supports (i.e- transportation, food, housing, employment, etc), and or other community resources as appropriate.
Essential Functions :

Collaborate with practice and health care teams Outreaches individuals recently served by the SVMC Emergency Department Conducts relevant screenings. Supports provider treatment plan and promotes adherence to physician/provider recommendations and instructions. Serve as a community referral resource within the practices.. Identify service delivery problems and potential for effective management intervention. Maintain annual competencies for role. Document all communication with individuals in Athena. Work at different office locations as needed. Perform all other duties as assigned.Competencies:
