Job Description
Care Coordinator
Transports clients and families to appointments and/or community resources, when needed.
Per grant requirements, completes surveys and/or assessments with clients and families and reports outcome data to program's Team Leader.
Provides case management, care coordination, and informal counseling for clients and families enrolled in program.
Evaluates and assesses the needs of the clients and families.

Summary:
Provides support to community youth and their families including informal counseling, care coordination, and overseeing the daily operations of the program's services.Responsibilities:
Develops and maintains daily activity schedules.Transports clients and families to appointments and/or community resources, when needed.
