Job Description
Hybrid Patient Safety Observer
Responsible for timely and accurate documentation of patient activity, interactions, and communication to the clinical team according to policy, protocol and/or standard of work in the electronic medical records.
When performing one-on-one monitoring at the bedside, remains at bedside of assigned patient at all times unless relieved by an appropriate staff member.
When performing one-on-one monitoring at the bedside, identifies and removes hazards from the patient and patient's environment, and reports as appropriate. May provide patient care, and/or assist assigned patient with performing activities of daily living, as delegated by supervising RN.Completes hourly rounding log for assigned patient for in-person duty.
During remote monitoring, verbally redirects the patient from engaging in at risk behavior. Contacts appropriate nursing staff if patient requires direct assistance.
Participates in the collaborative identification and reporting of patient safety issues and assures patient environment safety. Immediately follows the departmental protocol/guidelines and seeks assistance from the clinical team when patient appears to pose a threat to self and/or others.
Responsible for providing thorough hand-off report utilizing standardized electronic medical review for transition of duties.
Performs continual visual and digital 2-way audio surveillance of up to twelve (12) patients, on up to three (3) monitors which communicates directly with assigned patients via observation software.

Ensures safety and injury prevention for patients at high risk for falls, elopement, and other at-risk behaviors including disruption of treatment through continual visual surveillance via remote monitoring system and/or one-on-one in person patient monitoring under the supervision of a Registered Nurse.Responsibilities:
Consistently communicates with clinical team based on observation feedback of patients and waits for replacement prior to vacating the monitoring station for any reason to ensure constant patient observation remains intact.Responsible for timely and accurate documentation of patient activity, interactions, and communication to the clinical team according to policy, protocol and/or standard of work in the electronic medical records.

Complexity of Work:
This role requires critical thinking skills, high focus and attention to detail. Able to effectively communicate in English, both verbal and written., Decisive judgment, the ability to work in a stressful environment and take appropriate action, and the ability to build and foster positive relationships. Basic computer operation skills, and able to maintain medical records. Ability to pay strict attention to patient and surrounding environment for any other potential risk factors. Efficient in multitasking and managing the various job duties described. Knowledgeable and supportive of patient and Memorial Healthcare System (MHS) confidentiality along with related safety policies and procedures. Able to demonstrate flexibility and competency to provide patient care throughout the South Broward Hospital District to a variety of patient populations both virtually and in person.Required Work Experience:
No prior work experience but basic computer knowledge required. Previous hospital/clinical/direct patient care experience preferred. Knowledge of medical terminology preferred.Working Conditions and Physical Requirements:
