Minimum Qualifications:
Doctor of Pharmacy (Pharm.

D.) degree from an accredited institution. Active pharmacist license in the state of practice..
Responsibilities:
Review and interpret medication orders to ensure accuracy and appropriateness. Provide medication therapy management and pharmacoeconomic evaluations to optimize patient outcomes. Counsel patients on medication use, potential side effects, and adherence strategies. Collaborate with healthcare teams to develop and implement patient care plans. Administer immunizations and manage pharmacy benefit programs.
