Category Pharmacy - Staff Pharmacist Job Type Contract A home infusion clinical pharmacist plays a vital role in preparing, dispensing, and monitoring complex intravenous (IV) and subcutaneous medications for patients receiving care at home. This position serves as the central hub of a multidisciplinary team, ensuring safe medication use through sterile compounding, dose adjustments, and comprehensive patient education.

Responsibilities:

Prepare specialty pharmaceuticals and IV solutions in a sterile, ventilated environment adhering to all state and federal regulations. Assess lab values, perform pharmacokinetic dosing calculations (e.g., vancomycin), and monitor Total Parenteral Nutrition (TPN) therapy. Provide detailed counseling to patients and caregivers on specialty medications, infusion pumps, sterile techniques, and catheter maintenance. Collaborate with prescribing physicians, home health nurses, and pharmacy teams to coordinate patient care and minimize hospital readmissions. Maintain compliance with State Board of Pharmacy regulations, HIPAA, FDA, DEA, Medicare, ACHC, URAC, OSHA, and other relevant regulatory requirements. Manage pharmacy inventory, including refrigerated products, and prepare medications and supplies for safe, timely home delivery. Utilize CareTend system for clinical and operational documentation.

Qualifications & Desired Experience:

AZ and TX pharmacist licenses current and in good standing. 1 to 3 years of experience in infusion specialty pharmacy or hospital pharmacy with primary responsibilities in an IV room. Experience with IVIg therapy is a plus. Familiarity with specialty therapy payer requirements, reimbursement processes, and contract obligations. Strong communication and collaboration skills to work effectively with multidisciplinary teams and patients.

Monday - Friday, 8:30 am to 5:00 pm On-call rotation may be required
