Job Description

Every day you will rotate through diverse clinical settings, providing direct patient care under supervision. You'll evaluate medication regimens, make evidence-based recommendations, monitor therapy, and educate. Responsibilities include interdisciplinary participation, research, and contributing to quality initiatives.

Self-motivation and adaptability are essential. Participate in all aspects of comprehensive clinical pharmacy services including rounds with medical teams, pharmacokinetic consultations, parenteral nutrition consultations, anticoagulation management, antimicrobial stewardship monitoring, medication reconciliation, discharge counseling, documentation of clinical interventions in the identified data base, and compliance with clinical pathways and protocols Performs all activities associated with meeting the goals and objectives of the American Society of Health-System Pharmacists (ASHP) accredited PGY1 residency program. Process prescriber orders in the identified pharmacy information systems in an effective, efficient, timely, safe and accurate manner to ensure the appropriateness of the indication, dose/frequency, route, interactions, dose adjustment, physical and therapeutic incompatibilities, side effects, duplicate therapies, and cost-effectiveness. Evaluate need for all non-formulary medication required. Provide direct oversight and timely and accurate verification of final product for all aseptic parenteral and non-parenteral medication preparation, dispensing and distribution in compliance with state, federal statutes and hospital procedures. Ensure doses are filled and delivered within established departmental guidelines in a timely and safe manner.
