Job Summary Join our dynamic healthcare team as a Clinical Pharmacist specializing in Diabetes Management, where you will play a vital role in optimizing patient outcomes through expert medication management and personalized care. Duties Provide comprehensive medication therapy management for patients with diabetes, ensuring optimal glycemic control and adherence to best practices. Conduct detailed patient assessments, including reviewing medical histories, lab results, and current medications to develop individualized treatment plans. Collaborate with multidisciplinary teams to adjust medication regimens based on patient response, physiology knowledge, and pharmacotherapeutics principles. Educate patients on medication use, potential side effects, lifestyle modifications, and self-management techniques to empower them in their health journey. Ensure pharmaceutical regulatory compliance by adhering to drug regulation standards and hospital policies during medication preparation, IV infusion compounding, and drug administration. Monitor for adverse drug reactions or interactions, Maintain accurate documentation within Electronic Medical Records (EMR/EHR) systems and participate in pharmacy information system (PIS) updates for seamless clinical workflows. Skills Extensive clinical pharmacy experience with a focus on hospital medicine, critical care, or emergency room environments. Strong knowledge of pharmacology, physiology, anatomy, and medical terminology essential for effective patient assessment and medication management. Proven expertise in managing medications within managed care settings and understanding drug regulation policies. Experience with IV infusion techniques, aseptic procedures, IV compounding, and pharmacy aseptic technique standards. Familiarity with hospital pharmacy operations including hospital experience at Level I or Level II trauma centers is highly desirable. Skilled in utilizing EMR/EHR systems and pharmacy information systems (PIS) for accurate documentation and clinical decision support. Ability to precept pharmacy residents or students while demonstrating excellent patient service skills and fostering professional growth. Knowledge of acute pain management protocols, oncology medications, pediatrics considerations, and post-residency practice experience enhances your versatility. Join us as a Clinical Pharmacist
